दवा के फुटकर-थोक विक्रेता, डॉक्टरों और कंपनियों का ऐसा गठजोड़ है कि दवा बाजार तरक्की की सीढ़ियां चढ़ रहा है। हाल यह है कि शिखर पर पहुंचने के लिए बीमारों की जेब से धन निकाल कर बंदरबांट किया जा रहा है। मेडिकल बाजार में अंकुश न होने के कारण मनमानी ऐसी है कि जो साल्ट नौ रुपये में 10 गोली मिल जाती है, उसे नब्बे रुपये में ब्रांडेड का टैग देकर बेचा रहा है। लगातार बढ़ रहे इस बाजार का तिलिस्म ऐसा है कि इस पर न तो सरकार अंकुश लगा पाई और ना ही अधिकारी। अंधेर तो यह है कि जब अफसर भी बीमार होते हैं तो उन्हें भी ब्रांडेड दवा ही खरीदनी पड़ती है।

 

डॉक्टरों को दवा कंपनियों की तरफ से मिलने वाले उपहारों को लेकर सुप्रीम कोर्ट एक याचिका की सुनवाई कर रही है। इस मामले में सुप्रीम कोर्ट ने केंद्र सरकार से जवाब मांगा है। दवाओं की बिक्री को लेकर कंपनियों और डॉक्टरों की गठजोड़ को लेकर एक याचिका में ऐसा दावा किया गया है जिसे सुनकर खुद जज भी हैरान है। याचिका में कहा गया है कि डॉक्टर किसी खास दवा को प्रिस्क्राइब करने के लिए कंपनी डॉक्टरों को करोड़ों रुपए के उपहार देती है।

 

उदाहरण के तौर पर अक्सर बुखार में दी जाने वाली एक कंपनी की दवा की बिक्री बढ़ाने के लिए डॉक्टरों को 1 हजार करोड़ रुपये के उपहार दिए गए ताकि उनकी दवा का प्रमोशन हो। इस याचिका में कहा गया है कि जो डॉक्टर उपहार लेकर दवा की सलाह देते हैं, उन्हें इसके लिए जिम्मेदार भी होना चाहिए।

अगर इस तरह का काम किया जाता है तो ना केवल दवा के ओवर यूज के केस बढ़ेंगे बल्कि इससे मरीजों के स्वास्थ्य पर भी विपरीत असर पड़ सकते हैं। इस तरह के घोटालों से मार्केट में दवाओं की कीमत और बिना मतलब की दवाओं की भी समस्या पैदा होती है। हो सकता है कि कोरोना महामारी के समय ऐसी दवाओं का ज्यादा ही प्रमोशन किया गया और अनैतिक तरीके से मार्केट में सप्लाई किया गया। दवा कंपनी और डॉक्टरों के बीच की सांठगांठ़ फार्मा कंपनियों द्वारा अपने उत्पादों की बिक्री बढ़ाने के लिए डॉक्टरों को रिश्वत और प्रलोभन के माध्यम से बढ़ती जा रही है।

 

चिकित्सा प्रतिनिधियों ने यह भी उद्धृत किया कि केवल 10-20% डॉक्टर ही एमसीआई आचार संहिता का पालन करते हैं, जबकि कुछ मामलों में डॉक्टर किसी उत्पाद को आगे बढ़ाने के लिए “प्रोत्साहन” की भी मांग करते हैं। न केवल एलोपैथी, बल्कि आयुर्वेदिक और होम्योपैथिक कंपनियों के चिकित्सा प्रतिनिधियों ने उच्च बिक्री लक्ष्यों को पूरा करने के लिए भारी दबाव में होने की बात कही है।

 

रिपोर्ट में चिकित्सा प्रतिनिधियों का हवाला देते हुए कहा गया है कि कंपनी के अधिकारी डॉक्टरों द्वारा उत्पन्न व्यवसाय की निगरानी भी करते हैं, जिन पर उन्होंने ‘निवेश’ किया है। फार्मा कंपनियां चिकित्सा प्रतिनिधियों के लिए प्रशिक्षण कार्यशालाएं या सत्र आयोजित कर रही हैं, जो वे जिस उत्पाद को संभाल रही हैं, उसके बारे में अपने तकनीकी ज्ञान को बढ़ाने के बजाय, बिक्री कौशल और ‘ग्राहक (डॉक्टरों) संबंधों के प्रबंधन’ पर अधिक ध्यान केंद्रित कर रही हैं।

इस साठगांठ की समस्या से निपटने के लिए नया कानून लाने की सख्त जरूरत है, जिसके तहत दवा कंपनियों को डॉक्टरों को ‘शोध, लेक्चरों, सफर और मनोरंजन के लिए दी जाने वाली राशि का खुलासा करना जरूरी होगा। एक ऐसी नैतिक संहिता की भी बात हो, जिसके तहत फार्मा कंपनियां डॉक्टरों को किसी भी तरह का उपहार, धन या दूसरी तरह के फायदे अपनी दवाइयों को बढ़ावा देने के लिए नही दे सकें और न ही वे ऐसी जगहों पर बैठकों या सम्मेलनों का आयोजन करें, जो मनोरंजन, खेल के आयोजनों या मौज मस्ती और अवकाश देने से जुड़ी हों।

 

अनैतिक तरीके अपनाने पर सजा देने की जरूरत पर भी जोर हो। डॉक्टरों और फार्मा कंपनियों की इस अनैतिक साठगांठ को तोड़ने के लिए केवल कानून ही कारगर हो सकता है। दूसरी ओर जन औषधियों को बढ़ावा देने के साथ-साथ जागरूकता अभियान चलाए जाने की भी जरूरत है, जिससे मरीजों को उपचार के नाम पर न तो लूटा ही जा सके और न ही उनके स्वास्थ्य पर कुप्रभाव पड़े।

What is worrying is that there is no law to punish those guilty of unethical conduct. As a result, patients are forced to buy expensive medicines. The central government is still sitting on a 2015 proposal to introduce a Uniform Marketing Practice Code for pharma companies, which provides for stringent penalties.

 

A year and a half ago, a draft of regulatory codes sent to the Law Ministry to rule out unethical practices under the Essential Commodities Act was rejected. Still, the health ministry, in response to a Right to Information report, said the draft being discussed poses a health threat to millions of people who fall prey to irrational prescriptions pushing drugs that could be dangerous to health. go.

 

The increased use of antibiotics is the main cause of antimicrobial resistance, one of the world’s biggest health threats. Bacteria naturally develop resistance to drugs over time, becoming superbugs, but large-scale or improper use accelerates this process dramatically.

 

There is a need for a mandatory code to identify and penalize unethical promotions on the part of pharma companies. Compulsory disclosure by pharmaceutical companies of drug promotion spending, drug testing in continuing medical education, health care is not just a consumer product. It is more than that. The ideal of putting patients first is rooted in our culture. This is a major drawback when the actions of large health systems, pharmaceutical companies, device companies and insurance companies can have a greater impact on patients than the actions of individual physicians.

 

To tackle the problem of this nexus, there is a dire need to bring in a new law, under which pharmaceutical companies will be required to disclose the amount paid to doctors ‘for research, lectures, travel and entertainment’. There should also be talk of a code of ethics, under which pharma companies cannot give any kind of gift, money or other kind of benefits to doctors to promote their medicines, nor should they organize meetings or conferences in such places. , which are associated with entertainment, sporting events or providing fun and leisure.

 

There should also be an emphasis on the need to punish for adopting unethical methods. Only law can work to break this immoral nexus of doctors and pharma companies. On the other hand, along with the promotion of public medicines, there is also a need to run awareness campaigns, so that patients are neither robbed nor their health is affected in the name of treatment.
